My question is, how were the Old Testament people saved? Based on Acts 4:12 it seems they'll have to wait until after the Millennium?

It's all one, Bob. Those who go before us in faith are part of the same ongoing work and revelation of God to mankind. We need them and they need us.
“And these all, having obtained a good report through faith, did not receive the promise, for God had provided some better thing for us, that they should not be made perfect without us” (Hebrews 11:39-40 MKJV).
“Now therefore you are no longer strangers and foreigners, but fellow citizens with the saints, and of the household of God, and are built upon the foundation of the apostles and prophets, Jesus Christ Himself being the chief cornerstone” (Ephesians 2:19-20 MKJV).
“Therefore it is of faith so that it might be according to grace; for the promise to be made sure to all the seed, not only to that which is of the Law, but to that also which is of the faith of Abraham, who is the father of us all.” (Romans 4:16 MKJV).
